Information you choose to provide
The request form collects your name, phone number, email address, state, coverage interest, planning goal, and preferred time for a call. It records the date and time of your request, your affirmative contact request, and the consent wording and version shown. Do not submit Social Security numbers, payment details, account balances, or medical information through this form.
Facebook, Instagram, and other social lead forms
When you submit an EverPact Life Insurance lead form inside a social platform, that platform collects the fields shown in the form and makes your submission available to Jack Cook. These may include your name, phone number, email address, state, and preferred time for a call. The platform may prefill details from your profile; review and correct them before submitting.
The platform may also provide the submission time and the form, ad, or campaign associated with your request. Your submission is stored in the platform’s lead-management tools and may be downloaded for Jack’s follow-up and recordkeeping. Native social leads are separate from website submissions and are not automatically copied into this website’s database. The platform’s own privacy policy also applies to its collection, storage, and use of information. These inquiry forms request manual phone and email contact; they do not enroll you in automated marketing texts or prerecorded calls.

Jack uses your information to respond to the insurance inquiry you requested, discuss available options, manage follow-up, and maintain records of your request. The website records campaign labels in the page address when present, such as the advertising source, campaign, and creative. This helps identify which ad led to an inquiry. Where information is handled
Website submissions are stored through the website’s hosting and database services and are accessible through protected site-management tools. To notify Jack, your browser sends the contact details and inquiry you supplied, your preferred contact time, and campaign labels to Formspree for storage and email delivery to Jack’s business inbox. Formspree may use connection information and security checks to prevent spam; its privacy policy describes how it handles data. Jack’s email provider also processes the notification. Your request does not enroll you in automated texts. If you later apply for insurance, the insurer or other provider will supply its own notices; this form is not an insurance application.
Your choices
You may decline to submit the form and contact Jack directly instead. To withdraw a contact request, correct your details, or request access to or deletion of information, email or call Jack using the details above. Some records may need to be retained to meet legal, regulatory, or recordkeeping obligations. Information is retained only as reasonably needed to respond to your request and meet those obligations.
Security and intended audience
Technical safeguards include encrypted website connections, server-side form validation, restricted management access, and submission limits. No transmission or storage system is perfectly secure. The site is intended for adults age 18 or older and is not directed to children.
Updates and external platforms
This policy may be updated as the site’s services change. The effective date will be revised. Facebook, Instagram, TikTok, LinkedIn, Nextdoor, and other external platforms apply their own privacy policies when you use their services.
